THE Mines Inspectorate has issued a safety alert after a water truck operator was killed on an exploration site.

According to the DME, the worker sustained fatal injuries when he was crushed between the vehicle he was operating and a pastoral property gate.

Preliminary investigations revealed the vehicle’s brakes were ineffective.

The Department is urging all mines to constantly inspect and monitor braking systems in heavy rigid trucks and ensure stringent maintenance procedures are in place.

The DME says defect reporting systems must be developed and implemented at mine sites and guidance should be provided for operators.
