Authorities investigate fatal industrial accident

A 50 year-old Trans Pacific Industrial Solutions contractor has died at Alcoa's Wagerup refinery.

The man fell down a shaft at the refinery on 2 September 2009 at about 6:00am.

The Department of Mines and Petroleum says it will leave no stone unturned as it investigates the fatal industrial accident.

The department's Director of Mines Safety, Simon Ridge says the man's death is tragic news for everyone in the mining industry.
